Copies of forms are included on this website to familiarize users with them. The Pre-Cruise Plan includes the Vessel Pre-Cruise Plan, Scientific Personnel Information, Berthing Plan, Equipment List, and Ship and Boat Release. If applicable, a Guide for the Use of Radioactive Material At Sea is made available. On foreign cruises a Research Cruise Prospectus and Foreign Clearance Request  is required. It is necessary to allow 45 to 180 days advance notice of cruise departure in order to obtain foreign clearance via the State Department. Foreign cruises require completing a cruise summary, to be returned to this office within thirty (30) days after the completion of the cruise, for submission to the State Department. This is a requirement in obtaining clearance, and compliance is necessary.

The following forms should be completed by the Chief Scientist before leaving the vessel. These forms are available from the Captain. Should the Chief Scientist need assistance in completing these forms, the Captain will be glad to help.
