Immunization Records

You received immunization information in the first summer mailing. It is crucial that you complete the on-line information and submit the necessary forms right away if you have not already done so. The forms are due in Student Health by June 30. It is North Carolina State law requiring current immunizations. You will be un-enrolled in Duke University if you do not complete the required immunizations and submit required forms.
