Consent to Photographing, Video and Audio Recording

Throughout the year, as part of our efforts to showcase the experiences of students at the Nicholas School, members of the school’s communication team and other professionals hired by the school will be shooting photographs, making videos and recording various activities and events for use on the school’s Web site and Facebook sites and in its publications and various promotions.

Please complete the following form indicating whether or not you are willing to have your photograph, a video or recording featuring you used in this manner. Once completed, please save the pdf and email to Donna Sell (donna.sell@duke.edu) If you are in a situation where someone is taking a photo, making video or recording an event, please indicate to him/her if you DO NOT wish to participate.
